SISUN Specialized Surgery

Extreme Eyelid Reconstruction

Extreme eyelid reconstruction is SISUN Plastic Surgery's scar reconstruction procedure that simultaneously improves
hook-shaped inner-corner indentation scars caused by previous incorrect medial epicanthoplasty,
the resulting out-fold double eyelids, depressed scars visible when the eyes are closed,
and thick, sausage-like double eyelids.
For eyes in extreme conditions where revision surgery is difficult,
total reconstruction corrects them to a normal eye appearance.

Extreme Eyelid Reconstruction Candidates

  • Severe out-fold scarring caused by thick, sausage-like double eyelids
and an excessively hook-shaped inner corner
  • Severe scar adhesion at the surgical site
    after multiple revision surgeries
  • Difficulty opening the eyes due to severe upper-eyelid hollowness
    and scar adhesion
  • Over-opened inner corners causing severe shape distortion,
    overexposed lacrimal caruncle,
    and indentation scars
  • Over-opened outer corners causing a D-shaped deformity,
    exposed red conjunctiva,
    and lifted mucosal scarring

Why Extreme Eyelid Reconstruction Is Challenging

    • Incisional Ptosis Correction (Revision)+Epicanthoplasty Reconstruction (3 months)

    • Crease Lowering (Incisional Ptosis Correction) (Revision)+Upper Epicanthoplasty+Lateral Canthoplasty + Lower Eyelid Surgery (1 year)

    • Incisional Ptosis Correction (Revision)+Epicanthoplasty Reconstruction (Revision)+Lateral Canthoplasty Reconstruction (Revision) (7 months)

    • Incisional Ptosis Correction (Revision)+Orbital Fat Grafting (1 year)

    • Incisional Ptosis Correction (Revision)+Axis Rotation (Revision)+Lateral Canthoplasty Reconstruction (Revision) (1 year 2 months)

    • [Right] Incisional Double Eyelid+Axis Rotation (Revision)+[Left] Epicanthoplasty Reconstruction (3 months)

    • Incisional Ptosis Correction (Revision)+Axis Rotation (Revision)+Lateral Canthoplasty + Lower Eyelid Surgery (Revision) (1 year 5 months)

    • Incisional Ptosis Correction (Revision)+Upper Epicanthoplasty (9 months)

Unlike standard revision surgery, extreme eyelid reconstruction involves
severe tissue damage and adhesion, difficulty opening the eyes from ptosis,
complications from incorrect canthoplasty, and multiple eyelid creases.
In these cases, surgery must proceed with limited remaining skin and tissue,
and functional problems often accompany the cosmetic concerns,
so the procedure requires a meticulous revision plan considered from every angle
and a highly advanced operation that mobilizes multiple complex surgical methods.

Extreme Eyelid Reconstruction by Type 1

Double Eyelid and Inner Corner Problems

The most common type of extreme-eye case is a patient who has both
sausage-like double eyelids and inner-corner problems at the same time.

These symptoms cannot be fully improved with double eyelid surgery
or the Double Line Technique alone,
so the procedure should be performed together with Epicanthoplasty Reconstruction or Upper Epicanthoplasty.

Canthoplasty Complications

Incorrect canthoplasty surgery (medial / upper / lateral / lower)
can cause tissue damage, shape deformity, scarring, and exposure of the lacrimal caruncle or conjunctiva,
which creates an unnatural hook-shaped appearance and aesthetically unfavorable eyes.

When functional problems such as dryness, redness, and pain are also present,
it can interfere with daily life.

To improve these issues, the cause of failure must be analyzed precisely,
then Epicanthoplasty Reconstruction or Lateral Canthoplasty Reconstruction should be performed to correct the complications.
