Before performing eye surgery, it is essential to first determine whether the issue is with the eye itself or the surrounding area.
However, since it is difficult for patients to distinguish this on their own,
it is important for a specialist to make a clear diagnosis.

For example, the eyelid skin may be sagging and covering the double eyelid,
but the patient may think they need double eyelid surgery because the crease is not visible.
However, double eyelid surgery is not the right solution.
In such cases, a brow lift or endoscopic forehead lift to elevate the eyelid skin
is the fundamental solution, and patients must be clearly informed of this.
In other words, achieving the patient's desired results through accurate diagnosis is the key to anti-aging surgery.

Case 1. Only Brow Lift performed without other surgeries

Upper Blepharoplasty + Brow Lift + Lower Blepharoplasty (4 months)

There is a hidden inner crease,
but it is covered due to eyelid drooping.
In this case, even double eyelid surgery
would not reveal the crease line,
so a brow lift should be performed
instead of double eyelid surgery.

The scar under the eyebrow has healed
to the point of being nearly invisible,
and the upper eyelid that was triangular
has changed to a rounded parabolic shape,

Case 2. Only Endoscopic Forehead Lift performed without other surgeries

The patient wanted a higher double eyelid crease line.
However, since the distance between the eye and eyebrow was short,
revision double eyelid surgery had limitations in raising the crease,
making an endoscopic forehead lift more suitable.
Many people may wonder,
"Why a forehead lift when I want a higher double eyelid crease?"
Let us explain with the progress photos below.

Endoscopic Forehead Lift (Day 9)

9 days after surgery.
There is still some bruising on the left eye (in the photo),
but the recovery speed of an endoscopic forehead lift
is considerably faster than typical double eyelid surgery.
By lifting the skin through the forehead lift,
the distance between the eye and eyebrow increased,
and you can see that the double eyelid crease line
has naturally risen.

Endoscopic Forehead Lift (Day 9)

Let's look at the forehead and eyes together.
Even without double eyelid surgery,
the same effect as double eyelid surgery was achieved,
and the entire face was overall lifted
through the endoscopic forehead lift.

While aging affects each person differently,
there are common symptoms that generally appear in the eyes.
Eyelid drooping, hook-shaped inner eye corners, and upturned outer eye corners causing a triangular eye shape,
loss of eyelid skin elasticity leading to multiple creases,
and fat loss in the upper eyelid area causing hollowing.
These various symptoms cannot be improved with a single surgery, requiring a combined surgical approach.

Case 1. Medial Epicanthoplasty + Lateral Canthoplasty + Lower Eyelid Surgery

The inner eye corner extends sharply,
and the upper eyelid has drooped
into a triangular eye shape.
For this patient, upper epicanthoplasty,
lateral canthoplasty, and lower eyelid surgery
were performed to improve the eye shape.

5 weeks after surgery.
Through upper epicanthoplasty,
the 45-degree area of the upper eyelid was lifted,
correcting the sharp inner eye corner
to a rounded shape.
Lower eyelid surgery expanded
the 45-degree area of the lower eyelid,
and combined with lateral canthoplasty,
the sharp triangular eye shape
was transformed into a soft crescent eye shape.

Case 2. Double Eyelid + Ptosis Correction + Upper Epicanthoplasty + Eyelid Fat Grafting

These are typical middle-aged small constricted eyes.
The left eye (in the photo) has a drooping eyelid
covering the double eyelid crease,
and the right eye (in the photo) has lost fat
in the upper eyelid area,
resulting in a sunken appearance.

2 months after surgery.
For small constricted eyes,
it is common to perform canthoplasty
to open the inner and outer eye corners,
but since the patient wanted minimal surgery,
only upper epicanthoplasty was performed.
Through upper epicanthoplasty,
the 45-degree area of the upper eyelid was lifted
to create a rounded parabolic shape.
Through incisional ptosis correction,
the drooping eyelid was excised
and unnecessary tissue was removed.
Eyelid fat grafting added volume
to the deeply sunken upper eyelid area.

Case 3. Double Eyelid + Ptosis Correction + Upper Epicanthoplasty + Lateral Canthoplasty (Revision) + Lower Eyelid Surgery (Revision)

The outer eye corner is raised,
while the inner eye corner bends into a hook shape,
creating a triangular eye shape.
The eyelid skin that has lost elasticity
is pulled downward by the tension
of the epicanthal fold at the inner eye corner,
resulting in multiple eyelid creases.
In such cases, rather than simply performing
double eyelid surgery alone,
a combined surgical approach is needed
to achieve higher satisfaction.

3 months after surgery.
Non-incisional ptosis correction was performed,
combined with upper epicanthoplasty
to correct the hook-shaped eye
and multiple eyelid creases.
To transform the triangular eyes
into soft crescent eyes,
lateral canthoplasty and lower eyelid surgery
were additionally performed.

Aging not only changes the shape of the eyes,
but the biggest concern is eye health. If drooping eyelids are left untreated,
symptoms such as corneal damage from eyelash contact, obstructed vision, decreased eyesight, and skin irritation from tear discharge
may occur. Therefore, before the condition worsens,
it is recommended to correct drooping eyelids through surgery.
