Should I get reconstruction or revision?

“ Are canthoplasty and reconstruction different procedures? ”
Canthoplasty and reconstruction both reshape the inner and outer corners of the eyes, so experience in both approaches is important when choosing a clinic. SISUN's Canthoplasty & Reconstruction Lab draws on experience in both procedures. We assess each patient's concerns and consider correction of complications, corner shape, eye-opening size, and overall appearance when planning surgery.
Lacrimal Lake
VS
Shape
Depending on the degree of lacrimal lake exposure and inner eye corner shape,Epicanthoplasty Reconstruction(Axis Rotation) or Revision Medial and Upper Epicanthoplasty (45-Degree Canthoplasty) is performed.
Axis Rotation+Non-incisional Ptosis Correction(Revision)+Lateral Canthoplasty + Lower Eyelid Surgery(4 months)
[Repeat Canthoplasty After Reconstruction] Medial Epicanthoplasty (Revision) (6 weeks)
Epicanthoplasty reconstruction inevitably makes the eyes appear smaller. Therefore, many patients fall into a vicious cycle of medial epicanthoplasty → reconstruction → medial epicanthoplasty again. To prevent this, a method of performing epicanthoplasty reconstruction and upper epicanthoplasty (45-degree canthoplasty) simultaneously was developed. This restores the inner eye corner to the originally desired ideal shape while preventing eye size reduction.

The inner eye corner angle and shape that determine beautiful eyes
The bent inner eye corner is straightened and the 45-degree angle is rounded to beautifully correct the inner eye corner shape while properly adjusting the degree of lacrimal lake exposure. This is SISUN's specialized surgical method.
If only the surface skin is reconstructed during epicanthoplasty reconstruction, the inner eye corner shape may distort or separate after surgery. To prevent this, a triple-layer reconstruction of ligament, muscle, and skin is performedwith meticulous surgery to ensure no distortion over time.
Since the skin around the eyes is very thin and sensitive, surgery is performed with minimal incisions toreduce concerns about post-surgical scarring.

Micro-incision to reduce incision site scar concerns
Inner-corner complications and an excessively high eyelid crease can be addressed together through reconstruction and double-eyelid revision.

When previous reconstruction has closed the inner corner too much, repeat canthoplasty can adjust lacrimal lake exposure and reshape an overly rounded corner into a more natural, pointed contour.
[Shape Correction] Medial Epicanthoplasty (Revision)+Upper Epicanthoplasty (45-degree Canthoplasty) (Revision)(2 months)
[Over-Reconstruction] Medial Epicanthoplasty (Revision)(2 months)
If medial epicanthoplasty or upper epicanthoplasty (45-degree canthoplasty) is performed too aggressively, not only will the lacrimal lake be excessively exposed, but shape distortion and scarring may occur. Therefore, the most important aspect during surgery isproperly exposing the lacrimal lake while correcting the inner eye corner shape.

In cases where the inner eye corner has a hook shape after medial epicanthoplasty or upper epicanthoplasty,hook-shaped canthoplasty or zero-mm canthoplasty isapplied to achieve the ideal shape.

Square shape (unnatural shape),Conjunctival exposure
Insufficient effect
After lateral canthoplasty or lower eyelid surgery, if square shape (unnatural shape), conjunctival exposure, or insufficient effect occurs,Lateral Canthoplasty Reconstruction or Revision Lateral and Lower Canthoplasty is performed.
Lateral Canthoplasty Reconstruction+Lower Eyelid Surgery(4 months)
Medial Epicanthoplasty+Lateral Canthoplasty (Revision)+Lower Eyelid Surgery(Revision)(8 months)
Lateral canthoplasty reconstruction inevitably makes the eyes appear smaller. Therefore, many patients fall into a vicious cycle of lateral canthoplasty → reconstruction → lateral canthoplasty again. To prevent this, a method of performing lateral canthoplasty reconstruction and lower eyelid surgery simultaneously was developed. This restores the outer eye corner to the originally desired ideal shape while preventing eye size reduction.

While resolving the unnatural outer eye corner shape and conjunctival exposure, the under-eye line is gently opened to expand the eye size. This is SISUN's specialized surgical method for creating a softer eye appearance.

Lateral Canthoplasty Reconstruction
Lateral Canthoplasty Reconstruction+Lower Eyelid Surgery
The most important point of lateral canthoplasty reconstruction and revision surgery isthe shape and angle of the outer eye corner.Square/C-shapes and uneven shapes are corrected to a pointed “>” shape whileprecisely adjusting the eye angle to achieve dramatic results with a natural eye appearance.
Lateral Canthoplasty Reconstruction+Lower Eyelid Surgery (Revision)(3 months)
Rather than simply making a straight-line incision and suturing during lateral canthoplasty or lower eyelid surgery, the tissue is anchored to the periosteum and capsulopalpebral fascia, providing strong fixation. There is no concern about the lateral canthoplasty re-adhering or the lower eyelid surgery loosening.
Lateral Canthoplasty Reconstruction and Dual Canthoplasty: See cases at least 1 year after surgery.
Lateral Canthoplasty + Lower Eyelid Surgery
If lateral canthoplasty or lower eyelid surgery is performed too aggressively, side effects such as square/C-shape distortion or conjunctival exposure may occur. We precisely assess how much adjustment is possible based on each individual's eye structure.Dramatic eye enlargement without side effects is achievable.
Non-incisional Ptosis Correction+Hook-shaped Canthoplasty+Lateral Canthoplasty + Lower Eyelid Surgery(1 year 4 months)
Lateral Canthoplasty (Revision)+Lower Eyelid Surgery(3 months)
Previous medial epicanthoplasty may leave excessive lacrimal lake exposure, a sharply hooked inner corner, or scarring. Repositioning the skin, muscle, and ligaments helps restore a more natural inner-corner shape.

For patients with limited improvement after medial epicanthoplasty, excessive closure after reconstruction, or an unsatisfactory inner-corner shape.

Excessive closure after epicanthoplasty reconstruction
If the outer corner has become squared off or C-shaped, the conjunctiva is exposed, or the contour is uneven, the ligament and conjunctival layers are repositioned to improve the scarring and exposed mucosa.
