What Is Permanent Upper Epicanthoplasty?

Non-incisional Double Eyelid + Upper Epicanthoplasty (3 years 7 months)

Upper epicanthoplasty is a procedure that lifts the upper eyelid at the 45-degree area connected to the inner eye corner.
When this area droops heavily, double eyelid surgery alone often fails to reveal the crease line,
which can make the eyes look tired or sleepy.
With permanent upper epicanthoplasty, the double eyelid crease can begin cleanly from the inner corner,
allowing correction to a natural in-out fold or out-fold
for a more defined eye appearance.

Upper Epicanthoplasty Ideal Candidates

  • Double eyelid crease
    is hidden at the
    inner eye corner
  • Eyes appear
    sleepy
  • Eyes look fierce
    due to epicanthal fold
    at the inner corner
  • Short vertical
    eye length

What Is Epicanthal Fold?

Epicanthal Fold Commonly Seen in Asian Eyes
One characteristic of Korean eyes is that the inner eye corners are covered by the Mongolian fold, also called the epicanthal fold.
This fold can cover the front part of the double eyelid crease, create a hook-like inner corner shape,
and partially hide the pupil, making the eyes look heavy or fierce.
In such cases, removing the epicanthal fold is essential for achieving a more ideal eye shape.
Various Forms of Epicanthal Fold for Upper Epicanthoplasty
  • Double eyelid crease hidden by the epicanthal fold

  • Hook-shaped inner eye corner

  • Fierce-looking eyes

  • Covered pupil

If double eyelid surgery is performed without removing the epicanthal fold,
the vertical tension of the fold can increase the risk of the crease loosening,
and the covered inner corner makes it difficult to create the desired crease line.

Upper Epicanthoplasty Surgical Method

Before Surgery

<Problem Analysis>
- The double eyelid crease line
fades toward the inner corner
- The eyelid covers part of the pupil,
creating a sleepy appearance

STEP01

Design considering the inner
eye corner and crease line

STEP02

After making the incision,
the tissue is lifted in the 45-degree direction
and secured to the fascia

After Surgery

Clear double eyelid crease line
from inner to outer corner

01

Rounding the Inner Eye Corner Shape (Drooping and Angular Inner Corners)

If double eyelid surgery is performed without removing the epicanthal fold,
the crease line can get pulled inward, leaving the inner corner droopy or overly angular.
In these cases, medial epicanthoplasty and upper epicanthoplasty remove the fold and reveal the starting point of the crease line.

05

Hook Shape and Epicanthal Fold Improvement

Epicanthal folds that extend long and create a hook shape at the inner eye corner
can be corrected through medial epicanthoplasty (or epicanthoplasty reconstruction) together with upper epicanthoplasty,
creating a clearer, gentler crescent-shaped eye appearance.

06

Dog-ear Deformity Removal

Dog-ear deformity can occur when double eyelid surgery is performed without removing the epicanthal fold,
or when a step-off and scarring form during suturing after surgery.
In these cases, medial epicanthoplasty (or epicanthoplasty reconstruction) together with upper epicanthoplasty
can improve both the inner corner shape and the crease line at the same time.

07

Removing Epicanthal Fold Between Double Eyelid Crease and Eye Line

The epicanthal fold between the double eyelid crease and eye line
can create multiple layers in the crease or weaken the fixation, causing the line to loosen.
Therefore, medial epicanthoplasty and upper epicanthoplasty release that tension and help create a clearer crease line at a more appropriate height.
