The patient wanted to improve the overall appearance of small, sharp-looking eyes.

The inner eye axis turned downward, creating a sharp-looking appearance. Axis Rotation was performed to create a pointed inner corner and a rounded 45-degree contour. Lateral and lower canthoplasty enlarged the outer eye area and softened the overall impression. The eye shape became rounder overall, with a soft crescent-like contour.

A previous poorly performed medial epicanthoplasty had created a hook-shaped inner corner, while the 45-degree upper-eyelid area was covered by an epicanthal fold, resulting in a harsh, sharp-looking eye shape.

Axis Rotation straightened the bent inner eye contour and adjusted lacrimal-lake exposure to an appropriate level. The 45-degree eyelid contour was also refined into a rounded shape. Lateral and lower canthoplasty expanded the outer eye area, making the eyes appear larger and improving the overall impression.

The double-eyelid line was refined into a semi-outfold shape, and the drooping outer eye line was also improved.

An epicanthal fold covered the 45-degree direction of the inner eye corner, making the double-eyelid line unnatural and leaving the inner corner pointed in a beak-like shape.

Upper canthoplasty released the 45-degree epicanthal fold, and non-incisional ptosis correction strengthened eyelid opening, creating a defined yet natural in-outfold double-eyelid line.
